DWCNewsline 27-10: Division of Workers' Compensation introduces optional Medical Unit qualified medical evaluator panel request forms

The Division of Workers' Compensation (DWC) Medical Unit has created two new optional forms for use when requesting an additional qualified medical evaluator (QME) panel or a replacement QME panel. The two new forms are designed to help streamline the process for issuing additional or replacement QME panels and will also help cut down on the current panel backlog in represented cases.

To help the Medical Unit staff more quickly identify requests for a replacement panel and requests for additional panels in a different specialty, DWC is encouraging the public to use these new optional forms rather than submit requests with only a cover letter or trying to adapt a form 105 or 106. When using these forms, parties are reminded to fill in all the required information and to provide all essential documentation.
